Pepsinogen (PG) is the precursor of pepsin that divided into pepsinogen I and pepsinogen II according to its biochemical and immunochemical properties. Therefore, serum PG I and PG II levels can be used as reliable indicators of gastric mucosa atrophy. PG secretion also decreases in atrophic gastritis due to the decrease of secretory capacity, so the decrease of serum PG may indicate the progression of atrophic gastritis.

Pepsinogen I (PG I) is secreted by the main cells of gastric fundus gland and cervical mucous cells. PG I mainly reflects the function of the gland cells secreted by gastric acid, and it will increase when gastric acid secretion is more; When gastric acid production decreases or the gastric mucosa glands atrophies, PG I level will decrease.

Pepsinogen II (PG II) is secreted by fundus, cardiac, pyloric, and duodenal glands. PG II is highly correlated with the mucosal lesions of the gastric fundus, so if PG II is abnormal, it will mainly reflect the decline of the cell function of the gastric fundus.

The ratio of PG I to PG II is more clinically significant, and its ratio is significantly correlated with the progression of gastric mucosa atrophy. The level of pepsinogen in serum can reflect the secretion of pepsin and the state and function of gastric mucosa. When the gastric mucosa is diseased, the content of pepsinogen in serum will also change.
