Clinical Significance of Fecal Calprotectin (FC) Testing

Calprotectin belongs to the S100 protein family and is a heterozygous calcium-binding protein with a molecular weight of 36kDa. It is present in the cytoplasm of neutrophils. Calprotectin is quite stable, and fecal calprotectin (FC) can remain stable at room temperature for 7 days, making it a reliable marker for monitoring intestinal inflammation.

Elevated levels of fecal calprotectin are associated with active inflammatory bowel disease (IBD) such as Crohn’s disease or ulcerative colitis and have been included in the guidelines for IBD in China. Fecal calprotectin testing can help clinicians differentiate between inflammatory and non-inflammatory bowel diseases in the lower gastrointestinal tract, such as irritable bowel syndrome (IBS). It can also be used to assess disease activity and evaluate treatment effectiveness and predict relapse in IBD patients. Additionally, it has some value in colon cancer screening and differentiating diarrhea causes. The clinical significance can be summarized as follows:
